Nursing Services
PathForward’s full-time nurse practitioner and registered nurse provide free medical care, follow-up care, and prescriptions for people experiencing homelessness in Arlington at both the Homeless Services Center and our new location at RPC on Columbia Pike. These two centers allow us to deliver comprehensive, wraparound support where it’s needed most. We stabilize clients and, when appropriate, connect them with long-term primary care providers in the community. As Arlington’s population ages, our service centers will increasingly serve as a hub for expanded services and aging-in-place support.
Mobile Medical Program
Our innovative Mobile Medical Program (MMP) advances healthcare equity for those experiencing homelessness or living in supportive housing. Our “Mobile Medical Dream Teams” are composed of a registered nurse, licensed clinical social worker and case manager. These care crusaders bring healing and hope as they provide free healthcare and wraparound support to people on the street as well as to our clients participating in our housing programs.
The MMP offers essential medical services such as health screenings, wound care, medication and referrals to specialists. We focus on improving overall physical and emotional wellbeing, increased health literacy and building trust and meaningful relationships. This program not only serves our clients but also offers valuable training to medical students, teaching them to approach vulnerable populations with respect and dignity.
Medical Respite Program
Our respite care aids those discharged from an area hospital but in need of additional recovery with no home to go to to heal properly. PathForward provides five private beds for 25 patients annually.
Referrals
We find help for people with substance abuse or mental health issues and make referrals to medical specialists, eye doctors and dentists.
